It's been a couple of years since this page has been updated. It has received a cleanup---removing information that I find to be unnecessary or redundant---and some minor updates. Note that the removed phone number has not been in use for at least a couple of years; I apologize for anyone who may have tried to reach me, however unlikely that is.
